1. Click on Services from drop down menu bar (above).
   
2. Click on Request an Investigation.
   
3. Complete the referral form which will show up.
   
4. Click on "send request "or submit request.
   
5. Print a copy for your records.

If you plan on using our website frequently, please register on the "Adjuster Login" This will allow you to track the progress and status of your referral.

You can either submit referrals by following the above procedure, or you can simply print a form from the referral page site, and send us the referral by 24 hour fax to 877-774-9689.
